Voters can apply for mail-in ballots by e-mail or mail.
- Print, fill out and sign a Voter's Registration Form & Declaration and provide a copy of your valid photo identification.
- Submit your signed Voter Registration Form (front and back), a copy of your Photo ID that contains your signature (e.g. driver's license), to:

If your application does not meet the identification requirements or is insufficient or illegible, applicants will be asked to submit their application in person.
Applying In Person
Starting Monday, October 7th, 2020, you may apply for a mail-in ballot in person at Melfort City Hall, 202 Burrows Avenue West. When you apply, you must bring an acceptable form of identification, either:
- a government-issued photo ID that shows your name and address (e.g. valid driver's license), OR
- two other pieces of identification - one that shows your name and another that shows your name and address (e.g. passport plus a utility bill).
